If you must cancel or reschedule your appointment, please notify the CU Dental Faculty Practice Clinic at least 48 hours before the scheduled appointment time. Keep in mind that we may need to discontinue treatment for patients who cancel or miss multiple appointments within a short period of time.

To help you save time at your first appointment, make sure you have the following materials and information ready:

  • Your photo ID.

  • Completed patient forms.

  • Your medical and dental insurance cards.

  • Current x-rays from within the past two years from your dental provider, which should be emailed to CUDT@ucdenver.edu (this email is not for referrals).

  • If you are receiving oral surgery services, you will need a copy of radiographic images, which your dentist may email to sdm-omfsdfp@ucdenver.edu.

  • A list of your current medications and dosage information.

Bill Payment

The Dental Faculty Practice Clinic accepts cash, checks (with ID), debit cards, and credit cards. We currently do not accept Health First Colorado (formerly Medicaid).

We are also an in-network provider for Delta Dental, but we accept other forms of out-of-network dental insurance as well. We are not accepting BEAM insurance at this time. We are happy to work with your insurance provider to help you understand your insurance benefits.

Patients are required to pay all fees at the time of their appointment, regardless of insurance status or whether or not the insurance plan covers the services provided. However, we are always available to help you with your insurance claims and paperwork. For your convenience, you can also pay your bill online.

Depending on your treatment, you may receive a bill from CU Medicine, UCHealth (University of Colorado Hospital), and/or the University of Colorado School of Dental Medicine.

If you have any questions regarding your bill, please call:

Pay Your Bill

Medical and Dental Insurance

Please bring your medical and dental insurance information to your consultation so you can be reimbursed as quickly as possible. We’ll work with you to maximize your insurance reimbursement for covered procedures.

If you receive medical treatment and/or do not have dental insurance and receive dental treatment, expect to pay out-of-pocket.

Private Insurance

As a courtesy to our patients, we will send a bill to the insurance company. We recommend requesting a pre-determination for treatment above $200. Insurance coverage disputes are between you and your insurance provider; the School of Dental Medicine does not intervene.

The dental and medical benefits plan is based upon a contract between your employer and the insurance company. If you have questions regarding your dental and/or medical benefits, it’s best to contact your employer or insurance company directly. While we will do our best to interpret your benefit plan, we cannot guarantee reimbursement coverage.

Please remember that regardless of your insurance coverage, you are fully responsible for all fees that our office charges. These fees must be paid at the time of your appointment. You must also pay the remaining balance after your insurance has paid its portion.
